Physicals on July 12th @ 8 am @ Seiling Hospital

Incoming 7-12th Athletes: mark your calendars for July 12th!

Sports Physicals have been scheduled for Tuesday, July 12th at Seiling at 8 am. The bus will leave Vici at 7:30 am.

🔴 Students will need 4 forms on the day of physicals, and all need to be signed by a parent.
✔️OSSAA Physical Form
✔️Concussion: Head Injury Acknowledgement Form
✔️Sudden Cardiac Arrest
✔️ Seiling Hospital Parent Consent Form

🔴Forms can also be picked up at the table in front of Mr. Nelson’s office beginning Monday, June 13th.
🔴Forms will also be available on July 12th at the bus, but they MUST be signed by a parent before your child leaves for physicals.
🔴Riding the bus is not required, and you may meet the buses at the Seiling Health Clinic at 8 am.
🔴 If you are unable to get a physical on the 12th, it will be the parents’ responsibility to schedule and obtain a physical for your child.
🔴 All incoming 7-12 athletes MUST have a current physical on file with Mr. Halderman BEFORE they are able to practice or play.
